We set up your portal
We’ll help you set up and log in to the Carbon Health RPM provider portal to find and review patient information.
We help you find qualified patients
We’ll work with you to find patients who are clinically eligible for this program. We'll notify both your staff and patients if RPM can be reimbursed by their insurance provider.
We help enroll your patients
We’ll work with you and your staff to enroll patients — both in-person and remotely.
We work together to monitor patients
Our software continuously monitors your patients’ blood pressure readings so you have visibility into trends, medications, automated escalations, and related care team interactions.
We simplify billing
We provide expert guidance on revenue cycle management, including identifying which CPT codes to bill during a patient’s course of care.
